Self Sufficiency Coordinator

Title: Self-Sufficiency Coordinator (SSC) - Empowerment Services
Classification: Non-Exempt
Reports to: Program Manager of Empowerment Services
Job Summary: The SSC will support the transition of program participants as they address identified barriers to self-sufficiency. Utilizing a case management model, the SSC will maintain a positive and productive relationship with the client base to coach, mentor, and facilitate activities that meet the Individual Employment Plan (IEP) and Department need.
Job Responsibilities:
- Lead intake and assessment phases, including ensuring the program participants cooperation with the complete employment-centered case management process.
- Maintain contact with participants through weekly appointments and engagement activities to track their progress to interview/ job readiness status
- Facilitates the individual’s involvement with the creation and implementation of an Individual Employment Plan (IEP), including coaching individuals on meeting career/ employment goals, managing expectations, program performance and emerging opportunities.
- Inputs and maintains accurate up-to-date individual activity/data into the tracking system designated by the agency.
- Creates and maintains individual files per assigned case load.
- Facilitates group sessions and workshops to build employment readiness skills
- Distributes attendance stipends and retention incentives to program participants
- Prepares and submits assigned weekly, monthly, and quarterly reports.
- Facilitates referrals for individuals to internal and external resources as needed.
- Occasional travel off-site to support training or other outreach engagement activities
- Seeks out recruitment opportunities and works with colleagues to build a calendar of recruitment events
- Represents the Agency at internal and external outreach events and recruiting activities
- Using motivational interviewing and trauma informed care techniques as appropriate to help guide program participants to goal achievement
- Other duties as assigned.
Working Conditions and Physical Requirements:
Standard office environment. Requires ability to operate telephone and personal computer for extended periods of time. Occasional travel, sometimes overnight.

Education and Qualifications:
- Associate Degree in Social Work or related field or equivalent knowledge/experience.
- Up to two 2 -years relevant documented volunteer or work experience with coordinating client supportive services.
- Demonstrated knowledge of working with vulnerable or underserved populations.
- Knowledge of community-based resources.
- Knowledge of 21st century career skills and workforce development strategies.
- Knowledge of developing teaching aids, instructional tools, or demonstrations.
- Knowledge of independent living skills and work standards.
- Knowledge of client services, coaching and training practices.
- Knowledge of practices in serving and working with hard-to-place individuals.
- Willingness to receive professional development and training at the agency’s discretion.
