ORGANIZATIONAL IMPACT

Yolo County Children's Alliance (YCCA) vision is to protect and empower children by strengthening families throughout Yolo County. We recognize that better futures for our children start by building thriving environments to live, learn, and grow. YCCA ensures everyone in Yolo County has access to the resources they need to build meaningful ties to their communities and each other. To accomplish this, YCCA has expanded on our foundational role as the designated Child Abuse Prevention Council (CAPC) of Yolo County. Today, we serve as an inter-agency coordinator, building networks of support throughout Yolo County to deliver programs funded by numerous community and government grants, as well as individual donations. Our work focuses on three family-strengthening pillars: Family Support, Parent Education, and Community Collaboration.

ROLE IMPACT

Your impact as the CalAIM Coordinator helps YCCA in meeting the needs of the communities we serve by overseeing and helping implement YCCA's CalAIM work (California Advancing and Innovating Medi-Cal), including compliance with our managed care health plan contract, DHCS PATH CITED round 2 funding, and other funding sources.

A DAY IN THE LIFE OF A CalAIM COORDINATOR

You will work with the Housing team leadership to implement the CalAIM program including overseeing and training YCCA staff in our efforts to serve clients/patients with complex situations who are at risk of or experiencing homelessness. Under the direction of the Housing Program Manager, the CalAIM Coordinator is responsible for the day-to-day coordination of YCCA's CalAIM related case management, services delivery, billing, and reporting, including training, supervision, and support of YCCA's CalAIM program staff. Daily duties may include:

  • Supervise, support, and coordinate the work of CalAIM program Housing Navigators
  • Lead planning and implementation of CalAIM program initiatives, including Community Supports for the following types of services: Housing Transition/Navigation Services, Housing Tenancy and Sustaining Services, and Housing Deposits.
  • With the Housing Program Manager, co-lead and provide support for external partnerships with managed care plans serving Medi-Cal beneficiaries in Yolo County, including Partnership Health Care Plan of California and Kaiser Permanente
  • Monitor and evaluate effectiveness and efficiency of YCCA's CalAIM service delivery system, structure, staffing, and administration. Identify and recommend alternative approaches or improvements. Implement agreed upon adjustments and changes
  • Maintains an awareness of state-level changes to the CalAIM program that require local implementation changes or updates
  • Organize and lead internal YCCA CalAIM meetings, including weekly team program meetings, Housing Navigator check-in meetings, and CalAIM and other pertinent trainings
  • Organize coordination meetings related to CalAIM program with Housing Program Manager and other YCCA Leadership staff
  • In coordination with Development Department, implement and support program reporting, including those required by DHCS, PATH CITED funding, our managed care plan partners, and other CalAIM funders or partners. Examples include quarterly reports, and staffing capacity reports for health plans.
  • In coordination with Finance Department and Development Department, implement and support entire CalAIM billing and documentation process including proper completion of Treatment Authorization Requests (TAR), services documentation, service frequency documentation, client documentation, and proper completion of billing documents, including coding
  • Contribute to grant reporting and invoicing
  • Supervise, train, assign and evaluate the activities of CalAIM staff
  • Coach, mentor, and support CalAIM Housing Navigators and other CalAIM program staff
  • As needed, support hiring, staffing, and budgets
  • Internally liaison with other departments, including finance, development, administration, operations, data, and other program teams
  • Must work YCCA sanctioned community events that assist in delivering program services to the communities we serve
  • Perform any other related assigned duties needed to help drive our Vision and fulfill our Mission

CHARACTERISTICS & SKILLS FOR A CANDIDATE TO SUCCEED IN THE ROLE

Must Have Skills/Qualifications

  • MS Word, Excel, Outlook, PowerPoint, and ability to work effectively in a secure database
  • Strong data management and reporting skills for quality assurance and for presenting data in reports, memos, charts, graphs, and other appropriate forms
  • Remain knowledgeable of developments in the field of program support, administration, and data management
  • Experience with and knowledge of CalAIM populations of focus is essential
  • Excellent written and verbal communication skills
  • A current CA driver's license and proof of auto insurance are required. Must have reliable transportation and be able to travel regularly within Yolo County and surrounding counties

Nice to Have Skills

  • Effective oral and written skills in Spanish, Farsi, or Russian is desirable
  • Familiarity with Yolo County is a plus
  • Understanding of or experience with the 5 protective factors
  • Understanding of or experience with trauma informed care
  • Knowledge of child welfare system, educational system, and issues that affect families

Essential Physical Requirements

Candidates must be able to perform tasks which involve the ability to lift up to 20 lbs. in sedentary to light work daily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the role.

Experience & Educational Background

Candidates must have at least 5 years' experience in the development, management, and evaluation of community health or closely related programs as well as a BA degree or above.
